What to expect at your 6-week ultrasound appointment
At 5 weeks into pregnancy things are so small there is very little to see on ultrasound. Even at 6 weeks it can be difficult to see an embryo with some people. We say 5 weeks plus because without being able to measure the embryo, we can only measure the mean sac diameter which is a combination of 3 measurements of the sac to gather an approximate date, but it isn’t as accurate and measuring the embryo.
At 8 to 14 weeks of pregnancy, you should be offered a pregnancy dating scan. It will let you know a more reliable due date and check how your baby is.
The pregnancy is calculated based on the average menstrual cycle which is 28 days. The first day of menstrual bleeding marks the first day of the menstrual cycle. Right in the middle of this picture perfect 28 day cycle is when ovulation occurs day 14 of the cycle and roughly 2 days before and after is when you are most fertile. This is why many fertility doctors recommend to have intercourse every 3 days if you really are trying to have a baby because the ovulation may occur a little earlier or later than you may think.
When calculating the weeks of pregnancy we include those two weeks from the first day of the LMP as this is the easiest part of the cycle to notice and to record. We also will enquire about the length of your cycle and whether it is regular or irregular. All of this is helpful in trying to establish the gestation. For example, if your cycle is regular and 33 days long we would expect your ovulation to have occurred later 5 days and an early pregnancy scan will most likely show a baby that measures 5 days less than you were expecting.
Now to answer your question, it may be that you ovulated later in the cycle or have a longer cycle. The other possibility is that unfortunately the pregnancy is not developing as expected at 7 weeks gestation and a follow up scan may reveal a miscarriage. We understand that this is an anxious time and to have to wait for 10 days seems very long but it is important to have this time in between scans to determine accurately what is taking place.
To answer this question we firstly go back to basics to understand your cycle. You may also like to read on the blog Pelvic Floor Physiotherapy: What to expect from… Physiotherapy. Perimenopause and menopause result in massive and often unwelcome changes in your hormones and your body.

If you feel lightheaded , especially when you get up from lying down, be extra careful. Dizzy spells are common, especially as your pregnancy progresses and you get bigger, since your heart has to work harder against the forces of gravity to get blood to the brain.
Try to stand up very gradually, in stages, from lying to sitting to standing. Dizziness can also occur if you have been standing for a long period of time, since blood may collect in your legs. Keep moving to encourage the blood to be pumped back to your heart. Alternatively, you may feel dizzy due to low blood-sugar levels. Other symptoms of low blood sugar include feeling sweaty, shaky, and hungry.
Even if you’re feeling or being sick, try to eat little and often to ensure that your blood-sugar levels remain stable. The last thing you may feel like doing is exercising , but it can play a significant role in alleviating and preventing pregnancy symptoms. Although a run through the park or a walk downtown may not sound as inviting as a cozy nap on the sofa, exercise is invigorating and the effects will last. Try to think of it as being active, rather than “exercising.

In early pregnancy, especially before 11 weeks, it is usual to have a trans-vaginal internal scan , where a probe is placed in the vagina. This gives the clearest and most accurate picture in early pregnancy. The person doing the scan spreads a special gel on your lower abdomen below your belly button and above the line of pubic hair. He or she then moves the scanner over the gel, sometimes pressing down, until the uterus womb and pregnancy can be seen.
That may give some information about your pregnancy, but it is less clear than an internal scan and that could possibly delay diagnosis. There is no evidence that having a vaginal or an abdominal scan will cause a miscarriage or harm your baby. If you bleed after a vaginal scan, it will most likely be because there was already blood pooled higher in the vagina and the probe dislodged it.
